Micro-Grant Award Request Form - Prisma Health-Upstate Foundation

The Prisma Health-Upstate Foundation Micro-Grant program is designed to provide grants of up to $5,000 to fund innovative, compassionate solutions that will enhance patient care and health outcomes in the Upstate. Our Foundation board of directors is excited to invest in the creativity and insight of our frontline teams, knowing that the best ideas often come from those closest to the work.

Please remember these important guidelines when submitting your request:

Individual awards will not exceed $5,000
Applications will be accepted October 1 – November 2, 2026 for the FY27 grant cycle (requests received outside of this window will not be considered)
Applications will be reviewed by the Foundation Board of Directors in January
Funds will be made available to selected applicants in February and must be used by the end of the fiscal year
Award recipients are required to complete and submit an impact report outlining how the funds were used and the impact of the funding
